Predictive biomarkers for PD-1/PD-L1 blockade in NSCLC

Background

Across 47 screened papers, tumor PD-L1 expression (TPS ≥50%) remains the most cited enrichment criterion for first-line pembrolizumab, though TMB-high and STK11/KEAP1 co-mutations modify response in ways PD-L1 alone does not capture.

Predictive biomarkers

KEYNOTE-042 and CheckMate 227 support PD-L1 TPS stratification, while blood-based ctDNA clearance at week 6 shows emerging predictive utility. Evidence is strongest for tissue PD-L1; liquid biopsy markers remain inconsistently validated across cohorts.

Gaps in the literature

Few studies report discovery and validation cohorts in the same biomarker paper. Prospective trials linking composite signatures to OS beyond single-analyte cutoffs are still sparse.

How Motif's literature review pipeline works in practice

How does a Motif literature review work?

You ask a research question in plain English. Motif searches PubMed, PubMed Central, and Europe PMC, screens candidates against your objective, extracts structured biomarker associations from full text, and synthesizes a cited thematic narrative you can export as Word, Excel, CSV, or JSON.

How does Motif screen papers for relevance?

Before full-text retrieval, Motif runs a relevance gate on each paper's title and abstract against your research objective. Papers without enough abstract text are excluded; borderline cases are recorded in search provenance so you can audit what was screened out. For formal systematic reviews, a separate dual-pass eligibility tool is available.

Does Motif produce a PRISMA systematic review?

Not by default. A Motif literature review is a thematic narrative survey from papers in your knowledge graph, with no PRISMA flow diagram, dual screening counts, or risk-of-bias tables. For a formal PRISMA systematic review, you run search provenance, eligibility screening, and risk-of-bias assessment first, then synthesize as a systematic review. For grant backgrounds and scoping reviews, the literature review is the right starting point.

What databases does Motif search?

Literature search queries PubMed, PubMed Central (PMC), and Europe PMC. Preprints can be included via Europe PMC when enabled. Motif does not search Embase, Web of Science, or Cochrane as literature sources.

How does Motif score evidence quality?

Each extracted association gets a GRADE-adapted evidence certainty score based on study design, sample size, and reporting quality, surfaced as High, Moderate, Low, or Very Low in the UI. Association sentences link to source PMIDs with supporting quotes or figure interpretations, so you can verify every claim against the original paper.

What are the limitations?

Motif screens on title and abstract before reading full text, so relevance judgments are only as good as the abstract. Paywalled PDFs without open access may not be retrievable. Scope depends on your plan: Starter searches up to 5 papers per query, Pro up to 40. Meta-analysis requires at least three studies with comparable effect sizes. Researchers should verify key findings in the source papers.
